What is Frailty?
The ‘frailty fulcrum’ is an animated model for frailty that has been developed with these opportunities in mind. This model aims to provide a ‘common language’ for frailty that can be shared between individuals, carers and professionals. It offers an interpretation of frailty that is meaningful, relevant and sustainable for people living with the condition, throughout their journeys of care. Source.
What are we doing to support safer care for people living with frailty?
Following a request from the Patient Safety Collaborative Board, an expert reference group was arranged to discuss the potential scope of a frailty safety project in the region. Read the output report from the reference group on 16 June 2017.
