New Models of Care: prevention and early intervention event

Event summary

There is a significant amount of activity across the South West focussed on or around the GP surgery. This place based activity can cover social prescribing, GP workforce planning, patient safety, community development and integrated working amongst others. We believe that there could be transformative potential in these activities being aligned at a local level to both build resilient communities, reduce poor health outcomes and at the same time reduce the pressure and burden on GPs. Supporting this is technology and digital alongside new finance and funding models.

The SW AHSN finds itself in a privileged position to be at the centre of many similar discussions and we wish to use this position to bring people together.

This event is an opportunity to share and learn from existing practice across the South West and further afield.

It won’t be an ordinary event. We intend to run it as a drop in between 14:00 and 19:00 on the 18 July 2017, using innovative means to facilitate and structure the networking and learning opportunity so you get the best value for your time.

We will be running sharing opportunities via social media, then using the event to shape any communities of practice so you are able to take the learning and continue to expand and develop this.
